Discord Rich Presence for Jellyfin

Would love a plugin or a lightweight app that shows what I am currently watching on Jellyfin as a Discord Rich Presence status. Something like "Watching Breaking Bad S03E05 on Jellyfin" visible on my Discord profile. There are a few scripts floating around but nothing maintained and easy to install. Ideally it would run as a small background service on Windows and Linux and read the active session via the Jellyfin API. A Docker image would be a bonus.
